Rehabilitation protocols for joint-replacement patients, developed in conjunction with and individualized for referring surgeons; comprehensive wound care, with ready access to in-home CWOCN APN consultation; assessment, evaluation, education and direct care for patients with acute and/or chronic diseases and disabilities of all the body systems; infusion therapy; point of care PT/INR testing; and interventions to upgrade patients' overall safety and ability to manage their care at home, we will also arrange for private duty services, DME, supplies, oxygen and respiratory equipment, diabetic testing equipment and supplies, and other items needed to support patient care.
